The bill expands Medicaid coverage for inpatient psychiatric care for under‑65 beneficiaries to improve access to treatment, while increasing federal spending and imposing implementation burdens on states and potential service‑pattern disruptions for local providers.
Medicaid beneficiaries under 65 with serious mental illness or substance use disorders will gain Medicaid-covered access to inpatient and residential care in Institutions for Mental Diseases (IMDs) beginning Oct 1, 2025, increasing available treatment options that were previously excluded.
States can receive federal matching funds for behavioral health and long-term psychiatric services delivered in IMDs, reducing state budget pressure for those services.
Federal Medicaid spending and taxpayer costs are likely to rise because more IMD services become eligible for federal matching, increasing the federal budgetary commitment to behavioral-health inpatient care.
States and providers will face implementation and administrative costs and complexity to adapt Medicaid programs, provider payments, and oversight to cover IMD services.
Community non-IMD providers and local hospitals may experience shifts in patient demand that could disrupt existing service patterns and coordination of care.

Removes the Medicaid IMD exclusion so federal Medicaid can pay for items and services for IMD patients under 65 starting Oct 1, 2025.
Introduced September 30, 2025 by Shri Thanedar · Last progress September 30, 2025
Removes the Medicaid "IMD exclusion" so Medicaid can pay the federal share for items and services furnished to patients under age 65 who are treated in Institutions for Mental Diseases (IMDs). The change takes effect October 1, 2025 and applies to care provided on or after that date, and it includes conforming deletions of related cross‑references in the Medicaid statute. The bill does not appropriate money or add program conditions; it simply changes the statutory definition of "medical assistance" to allow federal Medicaid payment for IMD care for people under 65 beginning October 1, 2025.
