Official title: Reauthorize certain programs regarding rural health care.

The bill preserves and targets federal support for rural health programs through 2030 and increases local community involvement to improve access and relevance, but it raises administrative requirements, narrows allowable uses, and leaves funding levels uncertain, trading flexibility and budget certainty for accountability and targeted support.
Rural residents, including low-income individuals, will receive grant-funded programs targeted to local health care needs, improving access to services in underserved areas.
Rural communities will be formally involved in planning and ongoing operations of projects and networks, making services more relevant and responsive to local needs.
The program is reauthorized through 2030, maintaining federal authority to fund rural health outreach and network grants and preserving continuity of support for the next five years.
Hospitals and other grant applicants may face additional administrative requirements to demonstrate community involvement, increasing application burden and compliance costs.
Tighter specification of allowable uses could limit grantees' flexibility to fund other locally needed activities that don't clearly meet the new participation or targeting criteria.

Amends federal rural health grant law to require that grants for rural health outreach and network development be used to meet the healthcare needs of rural underserved populations and that those populations be involved in program development, operations, planning, and implementation. It also extends the authorization for the underlying program through 2030 (changing the authorization period from 2021–2025 to 2026–2030).
