The bill expands access to physical therapy in underserved areas and incentivizes PTs through loan repayment and Medicare coverage, but does so at modest federal cost and with implementation, funding adequacy, and resource-allocation risks that may limit or redistribute benefits.
Rural and other underserved patients (including Medicare beneficiaries, FQHC/RHC patients, and people with chronic conditions) will gain greater access to physical therapy services as NHSC recruitment/funding and explicit Medicare coverage increase local availability.
Physical therapists will be eligible for NHSC loan repayment and receive dedicated funding ($15 million) to reduce education debt, making practice in underserved areas more financially feasible and aiding recruitment/retention.
Medicare will explicitly cover physical therapy services provided by physical therapists at Rural Health Clinics and Federally Qualified Health Centers (effective Jan 1, 2027) and allow those clinics to bill Medicare directly, clarifying coverage and reducing billing barriers.
Taxpayers and Medicare will bear additional costs — including a $15 million appropriation and higher Medicare outlays from expanded coverage — increasing federal spending and budgetary pressure.
The $15 million appropriation and available NHSC slots may be modest relative to demand, so funding could be insufficient to meaningfully expand the PT workforce in high-need areas.
Expanding NHSC eligibility to physical therapists risks reallocating finite NHSC resources away from other high-need professions or areas, potentially reducing care for other underserved populations.

Official title: Amend the Public Health Service Act to provide for the participation of physical therapists in the National Health Service Corps Loan Repayment program, to amend title XVIII of the Social Security Act to expand Medicare Rural Health Clinic Services and Federally Qualified Health Center Services to include physical therapy services, and for other purposes.
