Application for Registration of Pesticide-Producing and Device-Producing Establishments (EPA Form 3540-8) and Pesticide Report for Pesticide-Producing and Device-Producing Establishments (EPA Form 3540-16)

FIFRA Section 7 requires that the Agency collect information on pesticide- production from pesticide-producing and device-producing establishments. The establishments initially apply for registration of their producing establishments (EPA Form 3540-8) and following assignment of the Establishment Number (Est.#), the producing establishment must report pesticide-production (EPA Form 3540-16) and annually thereafter on or before March 1st.
