Creates a federal Health Equity Innovation Grant Program that lets the Secretary award grants to faith- and community-based organizations to reduce health inequities and tackle chronic disease. Grants may fund medical and preventive services, expand culturally appropriate care and access, support community health workers and trusted messengers, and finance programs that address social determinants of health. The program sets eligibility rules (including hiring priority for entities that were active during a public health emergency), defines "community-based organization" by statute, and authorizes funding for fiscal years 2026–2030.
