Creates a time-limited Medically Tailored Home-Delivered Meals Program that lets a set of hospitals provide medically tailored meals and medical nutrition therapy to eligible Medicare Part A patients for up to six years. Hospitals must screen patients, deliver at least two tailored meals per day for defined periods, provide nutrition therapy, submit program data for evaluation, and follow payment and funding rules set by the Secretary.
