Expands Medicare coverage for diabetes outpatient self-management training by specifying an initial and annual number of training hours, allowing authorization by physicians or qualified non-physician practitioners, and removing the deductible for these services for items furnished on or after 2027-01-01. Creates a Medicare test model to evaluate virtual diabetes self-management training, requiring the Secretary to begin the demonstration by January 1, 2026, measure outcomes (health, utilization, medication adherence, costs), and consult stakeholders in model design.
