Requires CMMI to run a 5-year Medicare Part B demonstration called a Comprehensive Alternative Response for Emergencies that pays for emergency on-scene care when an ambulance is dispatched but the patient is not transported to a hospital. The model defines qualifying on-scene services, generally sets payment levels to be comparable to existing ambulance transport payments, and treats certain telehealth encounters as originating‑site visits so they can be billed when linked to an ambulance response.
The law also directs a Comptroller General report to Congress within four years on access, outcomes, regional differences, best practices, challenges, and recommendations, and makes two technical edits to the Social Security Act (section 1115A(b)).
